Penimex is a drug for the treatment of erectile dysfunction. The active component of the drug – sildenafil – helps to restore the impaired ability to maintain and achieve an erection of the penis in response to sexual arousal. The mechanism of action of the drug Penimex is the inhibition of a specific type 5 phosphodiesterase, which takes part in the breakdown of c-GMP in the cavernous (cavernous) body. Thus, under the influence of sildenafil, the level of c-GMP in the cavernous body increases and tissue relaxation and increased blood flow in the cavernous body are noted. Sildenafil does not have a direct relaxing effect on the corpus cavernosum.
Achieving and maintaining an erection when taking sildenafil develops only in response to sexual stimulation, in the absence of such a pharmacological effect of sildenafil does not develop.
With a single application of 100 mg of sildenafil, patients do not experience ECG changes, but some decrease in blood pressure is possible.
When administered orally, the peak plasma concentration of sildenafil is observed after 30-120 minutes, the average absolute bioavailability is 40%. Simultaneous administration of the drug with fatty foods leads to a decrease in the rate of absorption of sildenafil. The active ingredient of Penimex has a high degree of association with plasma proteins.
It is metabolized in the liver with the participation of the cytochrome P450 system (in particular with the participation of CYP 3A4 and CYP 2C9) with the formation of a pharmacologically active metabolite and a number of inactive substances. The half-life of sildenafil and its active ingredient is 3-5 hours. It is excreted mainly by the intestines. Some of it is excreted by the kidneys. In elderly patients and patients with severe renal impairment, the half-life of sildenafil increases. An increase in the half-life of sildenafil has also been observed in patients with impaired liver function.

Penimex ist ein Medikament zur Behandlung der erektilen Dysfunktion. Die aktive Komponente des Medikaments – Sildenafil – hilft, die beeintrachtigte Fahigkeit wiederherzustellen, eine Erektion des Penis als Reaktion auf sexuelle Erregung aufrechtzuerhalten und zu erreichen. Der Wirkmechanismus des Medikaments Penimex ist die Hemmung einer bestimmten Typ-5-Phosphodiesterase, die am Abbau von c-GMP im Schwellkorper (Schwellkorper) beteiligt ist. So steigt unter dem Einfluss von Sildenafil der c-GMP-Spiegel im Schwellkorper an und es wird eine Gewebeentspannung und eine erhohte Durchblutung im Schwellkorper festgestellt. Sildenafil hat keine direkte entspannende Wirkung auf den Schwellkorper.
Das Erreichen und Aufrechterhalten einer Erektion bei der Einnahme von Sildenafil entwickelt sich nur als Reaktion auf sexuelle Stimulation, ohne eine solche pharmakologische Wirkung von Sildenafil entwickelt es sich nicht.
Bei einer einmaligen Anwendung von 100 mg Sildenafil treten bei den Patienten keine EKG-Veranderungen auf, aber ein gewisser Blutdruckabfall ist moglich.
Bei oraler Verabreichung wird die maximale Plasmakonzentration von Sildenafil nach 30-120 Minuten beobachtet, die durchschnittliche absolute Bioverfugbarkeit betragt 40%. Die gleichzeitige Verabreichung des Arzneimittels mit fetthaltigen Lebensmitteln fuhrt zu einer Verringerung der Resorptionsrate von Sildenafil. Der Wirkstoff von Penimex hat einen hohen Grad an Assoziation mit Plasmaproteinen.
Es wird in der Leber unter Beteiligung des Cytochrom-P450-Systems (insbesondere unter Beteiligung von CYP 3A4 und CYP 2C9) unter Bildung eines pharmakologisch aktiven Metaboliten und einer Reihe von inaktiven Substanzen metabolisiert. Die Halbwertszeit von Sildenafil und seinem Wirkstoff betragt 3-5 Stunden. Es wird hauptsachlich uber den Darm ausgeschieden. Ein Teil davon wird uber die Nieren ausgeschieden. Bei alteren Patienten und Patienten mit schwerer Nierenfunktionsstorung verlangert sich die Halbwertszeit von Sildenafil. Eine Verlangerung der Halbwertszeit von Sildenafil wurde auch bei Patienten mit eingeschrankter Leberfunktion beobachtet.
